Pulmonology Survival Guide for Registrars

Welcome! -we trust that your time here will be informative and you will enjoy your experience in the unit. The objectives of the rotation are to learn the scientific basis of respiratory medicine & improve patient evaluation and management skills.
 

A.    PULMONOLOGY

2.    Referrals
Referrals are sent to A5.  Cases are firstly evaluated by the Registrar and thereafter seen and discussed with the Consultant on duty for the day.  Consultants will be available from 16H00 or as mutually arranged.  
You have to follow up patients in the wards and out patients, and finalize after discussion with the responsible Consultant.  Every patient receives a Lung Unit white card (stored in secretary’s office) which is placed in your folder.

As soon as patient data is completed, the final diagnosis is charted on the front of the card after which the secretary will file all documents.

3.    Lung function
We have a well established lung function facility under the management of
Mr Johan Mouton.  Everyone is expected to test their own lung function and to spend at least 2 weeks in the Lab to familiarize themselves with performing and interpreting  lung function tests.

4.    Theatre
The theatre falls under the direction of Prof Bolliger. It is your duty to see that patients are prepared, informed and ready at 09H00. Ensure that patient is NPM, consent signed, clotting profile normal (INR < 1,5, Hb > 10g/dl) working intravenous line, x-rays available and that a white card is filled in. On Tuesday afternoons, the Registrar working in the unit will help Prof Walzl.    

If the patient is not yet allocated to a Registrar, he/she (patient) is allocated to the Registrar who is on for theatre that day. Speak to Sr Oosthuysen/ Benting concerning the organization of the theatre.

5.    Clinics
Please write legibly! Summarize the patient’s problem & PRINT your name at the end to ensure continuity of care.

6.    Research projects
Research is a priority in the Unit, so feel free to liaise with the team; focus areas are: Lung Ca, Smoking cessation, TB and Asthma.  This is a good opportunity to plan or start your MMed project. We expect that every Registrar will prepare one good case study for publication in "Respiration".  

B.     INTENSIVE CARE

Prof E Irusen and Dr Usha Lalla are responsible for the Unit. Registrars’ exposure to the Critical Care Unit (ICU) is of extreme importance.  During your time in ICU, you will have to familiarize yourself with procedures such as intubation, CVP, arterial lines and insertion of chest drains.  Get to know patients who are assigned to you, anticipate problems and present them to the Consultant during the ward round.  
There is a call-list for Consultants - feel free to call them should you encounter any problems.  Due to staff shortages and funding, strict admission criteria are implemented in the ICU.  Good interpersonal relationships and consideration toward nursing staff, technologist, colleagues as well as the relatives of our patients are key for an enjoyable experience in the Lung Unit.
